Community health optimization is a key component to the “Second Curve” of population health. Last month, the American Hospital Association released a guide providing details and analysis for transitioning to the Second Curve and the importance of community-based solutions. (We provide a convenient download link for the guide on the webinar replay page.)

Additionally, the Second Curve can play an important role within a broader agenda for change including: guiding strategic direction, partnering with other organizations for efficiencies, reducing costs of medical care and generating revenue.

Watch this on-demand webinar replay as we:

  • Build an understanding of the importance of a community perspective in the second curve of population health
  • Identify new strategies for the design and implementation of population health programs by care delivery systems, public health agencies, and health plans
  • Walk through successful programs for population health and share best practices
